You can keep healthy by:
- Not smoking or getting help to stop - we offer stop smoking clinic - please book your appointment if you need our help
- Drinking no more than 14 units (female) or 21 units (male) of alcohol per week
- Taking regular exercise
- Eating a healthy diet
- Seeing your dentist regularly
- Following your doctor’s advice when taking any prescribed medicines
- Keeping any appointments at the surgery or at the hospital.
Many patients will see a doctor or nurse regularly, but others will not. Patients over 75 years who have not seen a doctor or nurse at all within the last 12 months, or patients between 17 and 75 years who have not seen a doctor or nurse at all within the last three years are encouraged to request a consultation for a review of their health.
Phoenix Family Care actively invites all patients aged between 40 and 75 who do not have any long-term conditions such as heart disease or diabetes for an NHS health check. In this check we look at overall health and family history of the patient. It is a very good review as we have now identified number of patients who were not aware that they have had a heart conditions or diabetes. This may be frightening for many people to hear but it is much better for your health to find out and have these conditions managed rather than living with undiagnosed conditions that can even become life threatening.
All patients over age of 75 are being also invited for an overall health check. As most patients are managed by their clinicians for their health, in this health check we are looking more on social aspect of quality of life and support we may be able to offer.
It is free so do not be scared to ask to be seen by your nurse for either of these health checks.
